What Is Adaptive Wellness Consultation?
A Place to Thrive provides customized adaptive wellness consultation and direct support services for organizations and families that support individuals with disabilities. These services are designed to enhance physical health, emotional well-being, social engagement, and overall quality of life through inclusive, ability-based wellness strategies.
Services may include any combination of adaptive fitness programming, nutrition education, social-emotional wellness support, staff or caregiver training, program development, and individualized wellness planning. Supports are tailored to meet the unique needs of each organization or family and can be provided on a short-term, ongoing, or flexible basis.
All services are delivered by an Adaptive Wellness Specialist with extensive experience supporting individuals with intellectual and developmental disabilities through inclusive, person-centered approaches.
Who Receives Adaptive Wellness Support?
Adaptive wellness consultation and services are available to:
Adult day programs
Group homes and residential programs
Schools and transition programs
Community-based organizations
Families and caregivers supporting individuals with disabilities
A Place to Thrive begins by meeting with organizational leaders, program directors, caregivers, or families to understand current needs, goals, staffing structures, and participant abilities. From there, a customized wellness support plan is developed to align with existing services while addressing gaps in wellness, engagement, and sustainability.
Organizations benefit from flexible, contracted wellness support without the need to hire additional staff, while families receive expert guidance and hands-on strategies that can be applied at home and in the community.

Funding & Contracting Options
Adaptive wellness services are offered through customized service agreements tailored to the needs of each organization or family. Agreements outline the scope of services, duration, frequency, and rates.
For organizations, services are contracted directly and invoiced on a monthly basis. For families, private pay options and flexible service packages are available. A Place to Thrive is also happy to collaborate with organizations seeking to align services with grant funding, pilot programs, or community-based funding opportunities.
